New Student Referrals

The referral process for new student enrollment in the Common Threads School Program has been redefined to provide a clear and consistent response across area school district teams, community service providers, and parents interested in learning more about our School Program.  Our policy now includes the following:

  • Parents, community service providers, and school district staff are welcome to schedule a time to meet with School Program Staff and tour the School Program building to learn more about Common Threads School Program as a potential alternative placement option. Click here to schedule a visit
  • Common Threads does not advocate for placement in the School Program, and will not provide assessment, evaluation, or recommendations for placement in response to parents or service providers seeking additional information or during tours.  Visit our Advocacy Resources page for a list of local organizations.
  • All inquiries made by parents and/or community service providers will be connected back to the home school district.

Referral Process

The referral process includes the following elements in order for enrollment to be considered:

  • Common Threads School Program must be contacted by a school district representative in order to proceed in the referral process
  • A minimum of one observation of the student in the current setting, to be conducted by Common Threads School Program staff
  • A scheduled appointment to tour the Common Threads School Program with the current IEP team (district staff & parents, as well as any other IEP team members)

  • A review of the student’s current IEP and Behavior Support Plan

An IEP team meeting is required in order to determine and/or change placement.  Attendance and signature of Director of School Services, Common Threads School Program required to change placement to Common Threads School Program  Revisions to IEP goals and objectives are made following the Intake & Assessment Phase of enrollment.

Intake & Assessment Phase

If Common Threads School Program is determined to be an appropriate placement option for a student, initial enrollment will begin in the Intake & Assessment Phase.

The Intake & Assessment Phase is offered either 8:30-11:00 or 12:00-2:30, Monday-Friday, for 4-6 weeks to complete the following activities:

  • Initial assessments by each related service provider (OT, SLP, Mental Health)
  • The Ziggurat Model framework elements
  • Integration into the Programming Strand most appropriate for the student’s needs and levels of functioning
  • Recommendation for continued placement
  • Development of suggested IEP goals for enrollment in the Common Threads School Program to present to the student’s IEP team
Post Intake & Assessment Phase

Following the 4-6 week Intake & Assessment Phase, an IEP team meeting will be held to review assessment results and recommendations for continued placement and programming.  If continued placement at Common Threads School Program is determined appropriate by the IEP team, the IEP goals and objectives will be revised to reflect ultimate goals for enrollment in the Common Threads School Program at this meeting.
